This document is a public offer pursuant to Articles 633, 641, and 642 of the Civil Code of Ukraine. It sets out the terms for the provision of psychological services by FOP Komarova Irina Volodymyrivna. Use of the services constitutes full and unconditional acceptance of this Offer.
1.1. Service Provider: FOP Komarova Irina Volodymyrivna.
1.2. Client: any individual who requests services and accepts the terms of this Offer.
1.3. Services: psychological and sexological consultations and therapeutic sessions provided remotely (online) or in person by prior arrangement.
1.4. The agreement between the Service Provider and the Client is considered concluded from the moment of booking a consultation and/or making payment.
2.1. The Service Provider delivers psychological services, and the Client accepts and pays for these services in accordance with the terms of this Agreement.
2.2. Services may include individual, family, or couples consultations, as well as psychotherapy and sexological counseling.
3.1. Booking a consultation is done via the website form, by phone, or through another agreed communication channel.
3.2. The Service Provider agrees with the Client on the format of the consultation: in person or online (video call via Zoom, Google Meet, etc.).
3.3. Session frequency is determined individually.
3.4. If the Client is more than 15 minutes late without prior notice, the Service Provider has the right to end the session at the scheduled time without refund.
4.1. Service fees are listed on the Service Provider's website or agreed separately with the Client.
4.2. Payment is made by bank transfer or another agreed method.
4.3. If a session is cancelled less than 24 hours before the scheduled time, the service fee is non-refundable.
4.4. If the Service Provider is unable to conduct a session for their own reasons, the Client has the right to choose a new time or request a refund.
6.1. The Service Provider is not liable for decisions or actions taken by the Client based on recommendations received during sessions.
6.2. The Client is responsible for the accuracy of provided data and timely payment for services.
7.1. The Client consents to the processing of their personal data in accordance with the Law of Ukraine "On Personal Data Protection".
7.2. Personal data is used solely for providing services and maintaining records.
7.3. The Service Provider does not transfer personal data to third parties without the Client's consent, except as required by law.
8.1. All disputes arising from this Agreement shall be resolved through negotiation. If agreement cannot be reached, the dispute shall be referred to a court in accordance with the applicable law of Ukraine.
9.1. This Offer remains in effect until withdrawn by the Service Provider or amended.
9.2. The Service Provider reserves the right to amend the terms of the Offer unilaterally by publishing a new version on the website.
